This Community Discharge Worker role is based within our specialist discharge service, and you will be working within a multi-disciplinary team to support individuals in their first six weeks post-discharge from hospital. This role is fast-paced, interesting, and always rewarding!

You will be working with a variety of individuals who may experience a range of diagnoses and needs; however, our goal is to improve patient experience and empower individuals to regain their independence outside of a hospital setting.

We have full, part-time, and flexible roles available.

Your role will be delivering quality support that enables people to achieve their outcomes by using and developing person-centred support plans. You will be able to manage risks and vulnerabilities, whilst promoting social inclusion, and maximising the opportunities for the people you support.

This Community Discharge Worker role would suit candidates with experience or an interest in developing skills in health and social care, mental health, as a Support Worker, Care Worker, Care Assistant, or similar role. Apply now to start your rewarding new career!

Additional Information

This Community Discharge Worker role will be a full-time 40-hour per week contract, Monday – Friday 09.00 – 17.30, or offered as 45-hours 09.00 – 18.30. The role is typically weekday; however, some limited weekend shifts may be needed for the demands of the service.

The role will be working with individuals being discharged from inpatient settings across the county and so the role involves supporting those across the West Sussex County.

Desired Skills

It would be desirable for you to have had experience of supporting a person(s) with a mental health condition and within a community-based services.
